Breezy Hayes, LMSW is the Program Manager for Substance Use Services for Centerstone’s southern region in Tennessee. She manages several programs that include co-occurring mental health and substance use programs for adults and adolescents across the organization’s nineteen outpatient clinics in Tennessee.
Ms. Hayes has served as the Project Director for three Substance Abuse and Mental Health Service Administration (SAMHSA) Adult Treatment Drug Court grants, which provides offenders access to substance use and mental health services who are enrolled in misdemeanor or felony recovery court programs. She has served as the Project director for a SAMHSA Medication Assisted Treatment – Prescription Drug and Opioid Addiction (MAT-PDOA) grant which enhanced access to medication-assisted treatment (MAT) services for persons with opioid use disorder (OUD) seeking or receiving MAT and decreased illicit opioid drug use & prescription opioid misuse at 6-month follow-up.
Ms. Hayes is the Project Director for a Substance Abuse and Mental Health Service Administration (SAMHSA) Certified Community Behavioral Health Clinic (CCBHC) which provides behavioral, substance use, and primary care screening and treatment. She also serves as a program manager for a Substance Abuse and Mental Health Service Administration (SAMHSA) Screening, Brief Intervention, and Referral to Treatment grant (SBIRT) which provides enhanced screening for substance use, treatment and coordination with primary care.
Ms. Hayes has over 10 years of experience working in community behavioral health working with marginalized populations. She received her MSSW in Social work from the University of Tennessee and her BS in Human services from Old Dominion University.
